Primary Care Nurse Practitioner (PCNP)

Description of the work

Omicron Clinique's LPCIs play an essential role in the delivery of primary care. They provide a full range of health services, including patient assessment, diagnosis, treatment, disease prevention and health education. They work closely with other members of the healthcare team to provide effective, comprehensive patient care.

Main tasks

  • Provide comprehensive clinical care and consultation, including assessment, diagnosis and treatment of patients within their scope of practice.
  • Manage and monitor chronic diseases, while working to prevent complications and tailoring therapeutic interventions to patients' needs.
  • Offer preventive services such as health screenings, vaccinations and advice.
  • Collaborate with physicians and other healthcare professionals to coordinate and improve the quality of care provided.
  • Participate in education, awareness and prevention activities for patients and their families.

Training and access conditions

  • Graduate degree in nursing with specialization in primary care.
  • Nurse practitioner title specialized in primary care (IPSPL) issued by the Ordre des infirmières et infirmiers du Québec (OIIQ).
  • Significant experience in primary care.

  • Excellent skills in clinical assessment, diagnosis, and therapeutic management in a primary care setting.
  • Ability to work independently, make clinical decisions and manage priorities in a dynamic environment.
  • Ability to collaborate with an interdisciplinary team and provide patient-centered care.
  • Bilingual (English and French).
  • Proficiency in Office suite software (Word, Excel, PowerPoint and Outlook).
