This spasm causes a complete or subtotal functional occlusion of the affected artery, leading to acute transmural ischemia of the corresponding myocardial territory. First described in 1959 by American cardiologist Myron Prinzmetal, this syndrome is distinguished from classic effort angina by the occurrence of crises at rest, frequently at night or in the early morning, the absence of a direct link with physical exertion, and the presence of ST-segment elevation on electrocardiogram during the crisis, unlike the usual ST-segment depression observed in stable angina. The coronary arteries may be angiographically normal or show non-obstructive atherosclerotic plaques.
