The term comes from Latin. <em>fistula<\/em> (pipe, flute). Fistulas can be congenital (embryonic malformations) or acquired, and are classified according to their anatomical course (enterocutaneous, rectovaginal, vesicovaginal, anorectal, tracheoesophageal, etc.), etiology (inflammatory, infectious, post-surgical, neoplastic, radiation, obstetric) and flow rate (low flow 500 mL\/24h for digestive fistulas). The common pathophysiology is as follows: a pathological process (chronic inflammation, infection, trauma, ischemia, irradiation, neoplasia) creates an abnormal communication between two structures, maintained by progressive epithelialization of the pathway - which explains why fistulas do not close spontaneously once formed. The factors that militate against spontaneous fistula closure are grouped under the acronym FRIENDS (Foreign body + Radiation + Infection\/Inflammation + Epithelialization + Neoplasm + Distal obstruction + Short fistula tract &lt;2 cm).
