It is the most common cause of acute kidney injury in children under 5 years of age in industrialized countries, but also affects adults in various forms. Contemporary classification distinguishes two pathogenetically distinct entities sharing the same clinical triad: typical HUS or STEC-HUS (HUS associated with Shiga-toxin-producing bacteria \u2014 primarily <em>Escherichia coli<\/em> enterohaemorrhagic E. coli (O157:H7 and O104:H4) \u2014 accounting for 90% of pediatric HUS cases + preceded by bloody diarrhea + self-limited in most cases + supportive care + NO antibiotics + NO plasma exchange \u2014 and atypical HUS (aHUS \u2014 atypical Hemolytic Uremic Syndrome) \u2014 secondary to a genetic or acquired complement dysregulation (alternative pathway) preferentially affecting renal endothelial cells + occurring at any age + high risk of recurrence + progression to end-stage chronic kidney disease without treatment + specific treatment with eculizumab (anti-C5 \u2014 terminal complement inhibition).
