What is a pilonidal cyst?

A pilonidal cyst (or pilonidal sinus) is a cavity formed under the skin, generally in the intergluteal cleft, near the tailbone. It often contains hair, skin debris, and sebum secretions. What causes a pilonidal cyst?

A pilonidal cyst results from the entrapment of hairs, skin debris, and sebum under the skin, usually in the buttock cleft. It primarily affects young adults, more often men.

How to recognize a pilonidal abscess?

A pilonidal abscess manifests as intense pain in the gluteal cleft, with a red, hot, and tense lump, sometimes accompanied by purulent discharge from a small skin opening. Fever, chills, or general malaise can be associated with it. Sitting positions often become very uncomfortable, and walking may be affected.

Can a pilonidal cyst heal without surgery?

An asymptomatic pilonidal cyst can remain dormant for a long time. However, in case of infection or recurrence, definitive treatment is usually surgical: urgent abscess drainage, followed by complete excision of the cyst and fistulous tracts. Without adequate surgery, recurrences are frequent and the progression to a chronic sinus is common.

What surgical techniques are used?

Several techniques exist: simple drainage (emergency for abscess), excision with directed healing (wound left open), excision with primary closure, or more complex techniques such as skin flaps (Karydakis, Bascom, Limberg) which aim to move the scar away from the midline to reduce recurrences. The choice depends on the extent, history, and prior scarring.

How to avoid recurrence after surgery?

Several measures reduce the risk of recurrence: rigorous local hygiene, regular hair removal in the area (gentle shaving, depilatory creams, or laser hair removal), keeping the area dry, avoiding prolonged sitting without breaks, losing weight if necessary, and wearing loose clothing.
