Is your card broken, lost, stolen, or expired? Order a form directly from the RAMQ — delivery by mail within 10 working days.

Since April 15, 2024, certain specialized nurse practitioners can care for individuals registered with the Family Doctor Finder.

To be eligible:

• Not already registered with a doctor or NP
• Hold a valid health insurance card
• To be domiciled in Quebec, current address with the RAMQ
• 14 years of age or older

The Quebec Health Record allows you to consult:

• Your list of medications registered with the pharmacy
• Medical imaging reports (ultrasound, X-ray, etc.)
• The results of your analyses (blood tests, urine tests, etc.)
• Medical services received and paid for on a fee-for-service basis by the RAMQ

Accessible on carnetsante.gouv.qc.ca or via the Carnet santé Québec mobile application.

Non-profit organization collaborating with Clinique Omicron to offer, without mandatory fees, care not covered by the RAMQ.

• Routine nursing care (dressings, health monitoring)
• Vaccinations for babies, adults, and travel health
• Blood tests and injections
• Health education and prevention workshops
• Support within the public health network

The ACPM provides forms designed to protect professionals who provide care to non-resident patients of Canada.

• Primacy of Canadian laws and courts in case of a lawsuit
• Simplification of procedures for non-resident patients
• Protection for physicians and healthcare organizations
