Professionalism, confidentiality and recognition
A care team that respects military experience
Our teams have been sensitized to the unique challenges faced by former military personnel. We act with respect, without judgment, and in complete confidentiality. Whether it's a chronic condition, a need for post-traumatic follow-up or assistance with loss of autonomy, we work closely with the family, caregivers and existing medical resources to ensure humane, rigorous and continuous care.
How clinical management works
A member of our team will schedule a home visit or introductory call based on your situation. No medical prescription is required. A caregiver or family member can make the request on behalf of the veteran.
An experienced nurse clinician performs a complete assessment: general health, pain, mental health, medication, mobility and functional needs. She identifies clinical priorities and determines whether a physician is required.
When a medical need is identified, a consultation with one of our doctors is quickly triggered - in person or via teleconsultation - to make a diagnosis, adjust a treatment or issue specialized referrals.
An adjustable care plan is proposed, taking into account the patient's evolving needs. Our team ensures follow-up in close collaboration with the family, caregivers and existing medical resources, including VAC where appropriate.
Assessment and certain treatments may be eligible for reimbursement through VAC, subject to prior authorization and conditions. We work with VAC to produce the required documentation to support your claim.
Our professionals understand the realities of active service, operational trauma and the challenges of transitioning to civilian life - without judgment, with respect.
Planning in a matter of days - no need for a family doctor or emergency room. Ideal for veterans whose condition requires rapid, structured care.
We act as a clinical hub between patient, family, GP, specialists and VAC - to avoid fragmentation of the care pathway.
The service adapts to the patient's mobility and preferences - home, RPA or clinic - without forcing the veteran to travel if he or she can't or doesn't want to.
All exchanges are protected in accordance with Quebec confidentiality standards. No information is transmitted without the patient's explicit consent, except in cases of emergency or legal obligation.
