Laboratory test

Serum A-1-antitrypsin

Serum alpha-1-antitrypsin is a laboratory test that measures the level of alpha-1-antitrypsin (AAT) in the blood. This protein, produced mainly by the liver, plays an essential protective role: it inhibits certain enzymes that, in excess, can damage lung and liver tissues. AAT deficiency is an inherited genetic condition that can progress silently for years.

What is this test for?

This test is prescribed when a doctor wants to detect an alpha-1-antitrypsin deficiency, a genetic condition that can lead to serious diseases such as pulmonary emphysema, even in people who have never smoked, or chronic liver diseases like cirrhosis.

When is it recommended?

Your doctor may prescribe this test if you have any of the following:

  • Unexplained pulmonary obstruction (shortness of breath, persistent chronic cough)
  • Chronic liver disease without an identified obvious cause
  • Family history of alpha-1-antitrypsin deficiency
  • Early emphysema appearing before age 45
  • COPD symptoms in a non-smoker or light smoker

Results Interpretation

The results must be interpreted by a doctor, taking into account your overall clinical situation.

Result Indicative rate Possible meaning
Normal 0.9 – 2.0 g/L Adequate protective rate
Bass Less than 0.9 g/L Possible genetic deficiency, liver failure, malnutrition
Raised 2.0 g/L Acute inflammatory reaction, active infection
ℹ️ AAT is an acute-phase protein: its level can be temporarily elevated during inflammation or infection, even in the presence of an underlying genetic deficiency. A complete clinical context is always necessary to interpret the results.

How do you prepare?

No special preparation is usually required for this test. It can be performed at any time of the day. Your doctor will inform you if specific precautions apply to your situation, especially if you are taking medication.
