The’hidrosadénite suppurée (HS) is a chronic disease which can be treated, even if it cannot be completely cured [1].

Hurley's three stages
- Hurley I : isolated abscesses, without fistula tracts or scars
- Hurley II : recurrent abscesses with some fistula tracts and scars, separate lesions
- Hurley III : diffuse involvement with multiple fistulous tracts and large inflammatory scar patches
- Visit classification guides the choice between topical treatment, antibiotic therapy, biologic therapy, or surgery
Principes généraux de la prise en charge
- Step-by-Step Approach : Start with simple treatments; add more depending on the response
- Combine medical treatment and lifestyle habits
- Anticipate the poussées and have a plan for prompt care
- Regular follow-up by a doctor (ideally in collaboration with a dermatologist)
- Include a psychosocial support systematic
To remember
- The’quitting smoking is the most easily influenced and best-documented factor
- Visit antibiotics are often used in treatment regimens lasting several months and in combination
- The’adalimumab and the secukinumab are the biologics approved by Health Canada for moderate to severe cases [2]
- Visit surgical approaches play a key role in established fistula lesions
- Visit psychological support is a valuable resource for dealing with a disease that is often overlooked
- In Quebec, the typical route begins in general practice, with a focus on dermatology for moderate to severe cases
Lifestyle
Visit lifestyle habits are not enough on their own, but they reduce the frequency and severity of flare-ups and improve the response to medical treatments [1][3].
The Most Useful Tools
- Smoking cessation — the factor most susceptible to influence; documented improvement in the number of episodes
- Avoid the friction repeated pressure on the affected areas (tight underwear, belts)
- Hygiène with mild, unscented soaps (syndets)
- Avoid the razors that cause irritation and the’épilation traumatique (waxing, harsh electric epilators) on the affected areas
- Réduire le stress and improve sleep
- Optimize health conditions associated with comorbidities (diabetes, depression, COPD)
Astuces concrètes
- Monitor skin folds (underarms, groin, under the breasts, between the buttocks) with special attention
- Sécher Gently pat your skin dry after showering, without rubbing
- Hydrate with a non-comedogenic cream between breakouts
- Soothe the painful areas with warm compresses
- Keep a journal flare-ups (triggers, pain, discharge, duration)
- Monitor les signes d’infection profonde (fièvre, érythème étendu, douleur intense) et consulter rapidement
Topical Treatments and Antiseptics
Visit topical treatments are the first line for mild cases (Hurley I, sometimes early Hurley II).
Available Tools
- Topical clindamycin For superficial lesions (apply twice daily)
- Antiseptics : chlorhexidine, benzoyl peroxide, applied topically or in soap
- Can be used in cycles or continuously, depending on the situation
- Often associés systemic treatments or topical treatments
- Well tolerated, with rare local side effects (irritation, dryness)
Wound Care
- Absorbent bandages non-adherent dressings for draining abscesses
- Cleaning Gently clean with saline solution or an antiseptic
- Bandages renouvelés selon l’écoulement (parfois plusieurs fois par jour)
- Avoid tight occlusive dressings that can cause skin maceration
- Recourir à une nurse or to a home health care service for complex or long-term care
- Recognize signs of a secondary infection (fever, increasing pain, spreading redness) and seek medical attention promptly
Antibiothérapie systémique
The’antibiothérapie systémique is used both in short courses of treatment for poussées et en cures prolongées pour réduire l’inflammation chronique [1].
Stratégies fréquentes
- Tétracyclines (doxycycline) as a course of treatment lasting several months, often 3 to 6 months
- Association rifampin + clindamycin as a 10- to 12-week treatment course for moderate cases
- Short-term treatments designed to treat an acute flare-up
- Métronidazole or other antibiotics, depending on the clinical presentation and resistance patterns
- Adaptation based on the history and comorbidities
- Follow-up side effects (gastrointestinal effects, photosensitivity, drug interactions)
- Risk of résistance bactérienne in the long term
- Effects indésirables Possible digestive and liver problems
- Précautions pendant la pregnancy : Several antibiotics are contraindicated
- Tracking organic in the case of long-term treatment
Hormone therapy
In some women, the’hormone therapy may reduce the frequency of flare-ups, especially when they occur in sync with the menstrual cycle.
The Main Options
- Contraceptifs combinés (estrogen-progestin combinations)
- Spironolactone, an antiandrogen used off-label
- Less commonly, finasteride or other antiandrogens
- Choice according to the hormonal profile, cardiovascular contraindications, thromboembolic risk, and preferences
- Evaluation complementary polycystic ovary syndrome (PCOS), which is often associated with
Precautions
- Contraindications for œstroprogestatifs : history of thromboembolism, migraine with aura, current smoking after age 35, etc.
- Adaptation based on the maladies associées (diabetes, high blood pressure, IBD)
- Décision partagée with the doctor, ideally in collaboration with the gynecology or dermatology department
Biological
- Adalimumab — the first biologic approved by Health Canada for moderate-to-severe HS (anti-TNF-alpha)
- Sécukinumab — most recent approval (anti-IL-17)
- For formes réfractaires, in a specialized setting
- HS modérée à sévère, usually Hurley II or III
- Answer insufficient first-line treatments (antibiotics, topical treatments, lifestyle changes)
- Évaluation préalable par un dermatologist or another authorized specialist
- Verification of the’absence d’infections actives (notamment tuberculose latente, hépatite B et C)
- Update vaccination before starting treatment
- Evaluation of comorbidities and contraindications
Modalités pratiques au Québec
- Administered by injection sous-cutanée (often from home after training)
- Regular follow-up by a specialist physician, including clinical and laboratory evaluations

Procedural and surgical approaches
Visit surgical approaches have a key role in the HS, particularly for the lésions fistuleuses installées and scar tissue [3].
The Main Options
- Drainage acute abscesses (incision and drainage), a measure to provide immediate relief
- Excision large scarred areas, sometimes followed by guided wound healing or a graft
- Laser (Nd:YAG, CO₂, hair removal laser) at certain specialized centers
When to Consider Surgery
- In case of fistulous tracts persistent despite medical treatment
- For scar tissue painful and chronic
- For abcès aigus a repeat offender
- With a surgeon familiar with dermatology (surgical, plastic, and general dermatology, depending on the location)
Suites opératoires
- Wound Care on a regular basis, often by a nurse
- Periods of recovery variables selon l’étendue de l’intervention
- Support postoperative pain
- Adaptation of the work and physical activities

Myths and misconceptions
False. L’HS n’est not contagious. The discharge may contain bacteria (secondary infection), but the disease itself is inflammatory and not contagious.
« Une chirurgie unique guérit la maladie »
False. Surgery treats lésions installées but it does not cure the underlying condition. Without medical treatment and lifestyle changes, relapses are possible. A combined approach yields the best long-term results.
« Les antibiotiques résolvent tout »
False. Les antibiotiques contrôlent l’inflammation and the bacterial component, but they do not cure the disease. For moderate to severe cases, they serve as a stepping stone toward biological or surgical treatment as part of a comprehensive strategy.
Frequently asked questions
À quel âge l’HS commence-t-elle généralement ?
L’HS débute le plus souvent à l’adolescence ou au début de l’âge adulte, between the ages of 18 and 30. It can appear earlier (during puberty) or later. Diagnosis is often delayed by several years due to a lack of awareness about the disease and the stigma associated with it.
There is a family component dans 30 à 40 % des cas.
Can you exercise with HS?
