Parents Plan
Home nursing follow-up
A nurse visits your aging parent at home. With their consent, you receive a report after each visit. If a doctor or nurse practitioner needs to step in, a private teleconsultation takes over. The plan is offered wherever the network provides home care services in Quebec.
For families and caregivers
Home nursing follow-up for your aging parents
Many aging parents prefer to stay at home, and their children want to know how they are doing, especially when they live far away. The Parents Plan meets this need: a nurse visits regularly, reviews your parent’s health and shares the essentials with you, with their consent.
This is common. According to the Institut de la statistique du Québec, more than one in four people act as caregivers in Quebec, mostly for someone aged 65 and over. The plan adds to the services your parent already receives and helps them keep living at home with regular health follow-up.
Simple for the whole family
How it works
- You tell us about your parent. Follow-up only begins with their consent.
- A nurse makes an initial visit at home, then returns one to three times a month.
- You receive a report after each visit, in plain language.
- If needed, a doctor or nurse practitioner takes over by private teleconsultation.
What you receive
What the plan includes
- One to three nursing visits per month at home: vital signs, medication intake, everyday questions.
- A written report, sent to the person your parent has chosen.
- A team that organizes next steps with you.
Billed separately, at the posted rate, only when needed: private teleconsultation, home specimen collection, home vaccination and home care.

Parents Plan options
It all starts with an initial home visit, at $175. The nurse meets your parent, reviews how things are going with them and recommends a visit frequency. You then choose the option together: the frequency follows your parent’s needs.
1 visit per month
249 $ per visit
Total: $249 per month.
For preventive follow-up, when your parent is independent.
2 visits per month
229 $ per visit
Total: $458 per month.
For closer follow-up, when the nurse recommends it.
3 visits per month
209 $ per visit
Total: $627 per month.
When the nurse notes a greater need at the initial visit. The frequency is reviewed every month.
In every option: the written report after each visit, with your parent’s consent, and travel to the home within the areas served. Visits completed are billed monthly. No commitment: you can stop at any time, without penalty.
Price per visit, plus taxes if applicable. Private service, not covered by the RAMQ. Spots open region by region: join the waiting list and we will write to you when a spot opens near you.
The home visit
What the nurse checks during the visit
Everyday health
- Vital signs: blood pressure, pulse, temperature, oxygen saturation, weight.
- Medications: intake, pill organizer, questions and effects noticed.
- Nutrition, hydration and sleep.
- Mood, memory and changes noticed by the family.
Life at home
- Mobility and risk of falls at home.
- Skin condition and minor wounds.
- Up-to-date vaccines and upcoming appointments.
- What helps your parent stay independent at home.
The content of the visit depends on your parent’s needs and the nurse’s scope of practice. The nurse does not make a diagnosis. If something needs to be assessed, the nurse reports it and suggests next steps: a private teleconsultation with a doctor or nurse practitioner, or the appropriate service. In an emergency, call 9-1-1. For a non-urgent health question, Info-Santé answers at 811.
Stay informed
A clear report for the family
After each visit, the person chosen by your parent receives a written report, in plain language:
- what the nurse observed;
- what was discussed with your parent;
- what needs to happen next, and who takes care of it.
Your parent stays in control. They choose who receives the information and can change their mind at any time. Their health information remains confidential.
For whom
The plan is a good fit when
- your parent lives at home and wants to stay there as long as possible;
- you live far away and want reliable news after each visit;

- your parent takes several medications and you want everything to stay clear;
- your parent does not have a family doctor and you are looking for a first point of contact for health.
